Fancy Little Paws contract 

The following is a contract between _________________ (breeder) and ______________________
and/or ___________________ (owners)
in regards to the following puppy: _______________________

 The breeder agrees to the following:
The breeder covers all genetic defects, which
are a result of the puppies breeding. This does
not cover illnesses or accidents that are not
genetic. Medical treatment must be medically
necessary. Breeder does not cover optional
services.

 Umbilical hernia's are covered under this
contract. The breeder will pay for repair of
umbilical hernia, however, this will only be done
at the time of spay/neuter while the puppy is
already under general anesthesia. The owners are responsible for all costs associated with the
spay/neuter. Puppies are covered against
contagious illness for the first 7 days after leaving
the breeder. In the event the puppies need medical coverage through this contact, a mutually agreed
upon vet must be chosen to provide care.

 For any and all above cases cost paid by the
breeder is limited to the purchase price of the
puppy, which is $________. $ __________ is
the maximum payable amount Cost paid by the
Breeder shall not exceed actual veterinary fees.
Optional services that are not medically
necessary are not covered under this contract.
The breeder agrees that if for medical reasons,
genetic defect or genetic health problems that
owners would prefer to return the puppy, a
replacement puppy will be offered to them as
soon as a litter of the same breed is available.
The puppy covered in this contract must be
returned to the breeder before a replacement
puppy will be given.

 The owners agree to the following:

The owners will continue medical care and shots for
the puppy listed above. This includes finishing puppy
shots. A series of three shots must be completed four
weeks apart including a rabies vaccine with the last
set of shots. once a year check ups and yearly shots
must also be maintained.

 Owners void this contract if they fail to maintain
their shots and check ups. The owners further agree
that they will never sell the puppies, give them away,
or place them in a shelter or rescue group. If the
owners need to re-home the puppies, this may be
done with the breeder's agreement. A new contract
must be signed at that time between the breeder and
the second owners. If no such agreement is made the puppy must be returned to the breeder regardless of
age.

 Owners agree to provide a indoor home. Puppies
may not be housed outside.

 We are always here for you, if you have questions
or ever need help with puppy purchased from us.
